LIFE STORY
It is essential that we “know our residents well” to provide a care service which is person centred and recognises the whole person. When we have knowledge of a residents’ past life and activity (as well as their present needs and wishes) we can engage in a more meaningful dialogue to maintain or improve their well being.

Residents may or may not wish to share this information with us and their choice to do so always comes first. By undertaking a ‘life story’, understanding of the whole person is achieved, which helps with the planning of social activity and important life events.
The assessment of social and life history is seen as a fundamental element of understanding people as individuals for the delivery of person centred care.
The life story can be provided by relatives or a staff member. Where a person is suffering from a degree of memory loss, friends and families are encouraged to complete the life story.
The Life Story should be completed to assist with the development of the resident's plan for social and recreational activity (with their agreement). Contribution to the 'Life Story' is from the resident, and where appropriate from their next of kin, family and friends.
